Getting started with PQRI

Needing to get info from pulmonary office that is currently submitting claims with the PQRI codes.  We are a practice with 8 physicians looking to start the process to receive the PQRI incentives, but are unsure how the process should take place.  One question is do we have to submit a letter nominating ourselves?  The info that we have found states this should have been done prior to 01/31/2010.  If we did not do this -- is it too late?  Also can we start this late in the game for this year, or just postpone until next year?  Also our physicians work in the hospital as well as office.  Do we just report on patients that are seen in office??  Thx in advance for any responses.
