I have completed quite a few of the provider-based attestations and have a few comments. The actual attestation form is supplied on the Medicare website. The form itself is actually quite benign; basically yes and no questions. The form is directed by the on-campus or off-campus criteria. What I found most difficult are the attachments that you need to send to both your FI and Medicare to support each one of those "yes" answers that you supply on the attestation. What they are basically asking you to do is prove that you are part of the operations of the "main campus," which for me is our hospital. I had to go down as far as our corporate bylaws for some of the proof they were asking for.
