Patient Satisfaction Survey

    Our practice has decided to create an online Patient Satisfaction Survey to offer to patients who visit our website. Our goal is to monitor and improve our patients experience with us through better customer service .  By implementing a new survey we are hopeful that  we will be able to produce  more effective results that will in effect produce positive change within our staff. 

    Currently, we are using a one page likert scale survey with several open ended questions but have not had much success using our results for anything. The new survey will be a link posted on our website that patients can just click and answer within a short time.

Does anyone have any suggestions?

1. What types of questions should we ask to get effective results? i.e. How do we evaluate our results to produce changes in our practice?
2. Has anyone had success with a survey that they are currently using? Online or paper.
3. If using an online survey, who is your vendor? Are you satisfied with their services?
    - Survey Monkey? Surveygizmo? Chekcbox?
4. Has your online survey been useful? Are people actually taking the survey?

Also, if you have any suggestions please let me know. I am interested in any advice to help us improve and would greatly appreciate it!
