Evaluate our front office staff and call center in relationship to providing optimum customer service

I have been asked by my physicians to have patients complete a survey or evaluation of their experience with the front office staff and/or the call center.
I have been looking for a tool or survey to do that, not having any luck finding one. Does anyone know of an available survey or tool for evaluating the front office staff & call center in relationship to customer service provided to the patient?

I like survey monkey. You can create your own surveys.
