EMR fundamental flaw

Electronic Medical Record systems (EMR), Electronic Health Records (EHR) systems are being produced with a fundamental flaw that is creating inefficiencies with providers.  Inappropriate auditing tools are being circulated and used to down code medically necessary services.   The EMR systems promote the use of physician note templates that offer no meaningful use.  The notes are robotic and if you ask a provider what information they rely when looking at a patient note they will always tell you they review the chief complaint, the interval history and the last paragraph of the assessment and plan.  The History of present illness, the review of system and exam offer no meaningful information about the patient condition. This information and more suggest the American Health Care system is creating a EMR, EHR  systems where 80% of the data is useless.   PQRI validates this concern since CMS is paying for data that should be part of the Evaluation and Management note.
